2025 STAFF FORMS

All hired Briarwood staff needs to submit health, clearances and other paperwork to work at camp. Links are below.

Different staff members will need to complete different forms. Please click on the button immediately below to see which forms must be completed.

• 2025 Staff Health Form (and other forms) – For all hired staff – Please click on “Sign in if you already have an account,” using your already created ID and password. Then go to Forms & Documents. Please call us if you are having problems or if need us to reset your password.
• 2025 Junior Counselor Permission to Medicate Form – For junior staff who takes prescription medication only.
• Department of Homeland Security I-9 form – For all hired staff (if you cannot download the fillable form, click here for a “regular” PDF form). This form should be brought to staff orientation – DO NOT email, fax or mail to Briarwood.
• IRS W-4 form – For all hired staff.
• PA Criminal Background Check – For all hired staff (select Submit a New Record Check, NOT New Volunteer Record Check).
• PA Child Abuse Clearance – For all hired staff.
• National FBI Fingerprint Check – For certain hired staff.

• Junior Counselor Certification of Residency and Clear Record – For certain hired junior staff.
• Certification of Transferred Valid, Existing Clearances – For certain staff who are providing clearances originally obtained for a different job.
• Working Papers – For certain hired junior staff only. These must be obtained from your high school.
